Our office prepares Illinois Statutory Short Form Powers of Attorney for our clients.
Each client receives an Illinois Statutory Short Form Durable Power of Attorney which relates to property, an Illinois Statutory Short Form Health Care Power of Attorney, and a Declaration which most people refer to as a Living Will.
Our office prepares the appropriate number of copies that are signed as originals of each document, and we have in-house ancillary documents which are prepared to help our clients be sure that the documents are distributed properly.
Our policy for many years has been to make sure a client(s) who does not have a Will nor any of these directives, to put together a "Will Package" which includes a Last Will and Testament along with these three documents. If the client(s) already has a Will, then we will prepare what they need as far as the Property and Health Care documents.
